New Patients

South Street Surgery welcomes new patients. If you would like to register with the Surgery please first of all check that you live within the practice area. Then complete an online form and bring your ID to reception.

Alternatively, you can download the registration forms below and bring them into the practice.  You will also need to bring with you photo ID (passport or driving licence) and proof of address (utility bill or tenancy/mortgage agreement).


Change of Personal Details

If you have any changes you wish to make to your personal details or address, please complete our online Change of Details form.

Temporary residents

If you do not reside the area, but you are staying in the area at least overnight or for up to 3 months then you can be seen at the Surgery as a Temporary Resident. This applies to any university students coming home for holidays/leave or anyone visiting a family member that lives in Bishop’s Stortford.

Overseas Registration

Please telephone the surgery and discuss with the receptionist. We are able to see you, however, you may have to pay for care. The rules relating to NHS entitlement for overseas visitors are complex. More information can be found on the Department of Health website.
