Please complete the online access form which is available to be downloaded from our website, or from reception. You will need to come into reception with your completed form, and a form of photo ID. Your new login details will then be issued to you. Please do contact reception if you have any queries about this.

SystmOnline Patient Guide

Booking Appointments

Due to the coronavirus pandemic all online appointments booking has been suspended in line with guidance from NHS England. SystmOnline is a secure facility for the booking of online appointments. As well as making online appointments, it has additional facilities for making changes to your personal details e.g. email address or telephone number and online ordering of repeat prescriptions. The following guidance gives further information and outlines our terms and conditions.

Repeat Prescriptions

Please nominate a pharmacy of your choice where you would like to collect your prescriptions from. Your repeat prescriptions may be ordered online. When ordering repeat prescriptions please identify in the “Message” box where you would like to collect the prescription from. The options are:

  • South Street Surgery
  • Bishop’s Park health Centre
  • Trinity Chemist
  • Williamsons Chemist
  • Boots (Bishop’s Stortford)
  • Tesco (Bishop’s Park)
  • Lloyds in Bishop’s Stortford Town Centre
  • Lloyds in Thorley

View Your Medical Records

When you log into the Online Services you will now be able to view part of your medical records including, current medication, repeat dispensing, any allergies and immunisations we have recorded. You will also be able to send a request to the practice to view your full medical record.

Inappropriate Use

We will be monitoring the use of this service and we are sure that you will find it useful. If, however, we find that any users are abusing the service, we will revoke access to the service and you will have to liaise with our reception team for services.

Examples we would consider as inappropriate include:

  • Sending inappropriate or abusive messages
  • booking appointments and not using them
  • booking appointments for other family members using your name
  • consistently booking inappropriate appointments with the doctors

Termination of Service

We reserve the right to terminate this service (or part of the service) without notice.

Changes to Service

Any changes to this service or to the terms and conditions will be posted on our website.

Register for Online Services

South Street Surgery offers access to certain aspects of your medical records.  Use this form to request access to the internet facilities offered by this practice.  For security reasons you will need to provide photo evidence of your ID such as a passport or photo driving licence before receiving your access codes.  You can only apply for yourself on this form or your child up to their 11th birthday. Once your child reaches the age of 11 your access to their records will cease and they will now have to consent. Further forms will need to be completed by yourself and your child. If access is given this will cease again on the day before they reach 16 and they will then need to apply for their own online access and record viewing. If applying for a child you will need to bring your photo ID and your child’s birth certificate.  When the form is completed please hand it in at reception with your ID and you will be issued with your online access ID and instructions.

The NHS App

You can also apply to access online services via the NHS App, for further information at www.nhs.uk/nhsapp

The NHS App is a national initiative to give patients better access to their own healthcare and NHS services. With other services also built-in, the NHS App should become the go-to place for patients. The NHS App can be used from anywhere, any time of day or night.

eConsult

The eConsult tool available via our website enables you to send a message to the practice at any time or find advice about common health problems. If submitted by 12.00pm we will aim to respond the same day otherwise we will respond by the end of the next working day. It may not always be necessary for the clinician to speak with you. eConsult is also available via the NHS App.
