Please be aware that the Practice changed our clinical system to SystmOne in April 2016. All patients who had access to online services for repeat prescriptions and booking appointments under the previous system, EMIS, will need to register for online access again if they wish to have this in SystmOne. Please complete the online access form which is available to be downloaded from our website, or from reception. You will need to come into reception with your completed form, and a form of photo ID. Your new login details will then be issued to you. Please do contact reception if you have any queries about this.
SystmOnline is a secure facility for the booking of online appointments. As well as making online appointments, it has additional facilities for making changes to your personal details e.g. email address or telephone number and online ordering of repeat prescriptions. The following guidance gives further information and outlines our terms and conditions.
Your repeat prescriptions may be ordered online. When ordering repeat prescriptions please identify in the “Message” box where you would like to collect the prescription from. The options are:
- South Street Surgery
- Bishop’s Park health Centre
- Trinity Chemist
- Williamsons Chemist
- Boots (Bishop’s Stortford)
- Tesco (Bishop’s Park)
- Lloyds in Bishop’s Stortford Town Centre
- Lloyds in Thorley
View Your Medical Records
When you log into the Online Services you will now be able to view part of your medical records including, current medication, repeat dispensing, any allergies and immunisations we have recorded. You will also be able to send a request to the practice to view your detailed coded medical records.
We will be monitoring the use of this service and we are sure that you will find it useful. If, however, we find that any users are abusing the service, we will revoke access to the service and you will have to liaise with our reception team for services.
Examples we would consider as inappropriate include:
- Sending inappropriate or abusive messages
- booking appointments and not using them
- booking appointments for other family members using your name
- consistently booking inappropriate appointments with the doctors
Termination of Service
We reserve the right to terminate this service (or part of the service) without notice.
Changes to Service
Any changes to this service or to the terms and conditions will be posted on our website.
Register for Online Services
South Street Surgery offers access to certain aspects of your medical records. Use this form to request access to the internet facilities offered by this practice. For security reasons you will need to provide photo evidence of your ID such as a passport or photo driving licence before receiving your access codes. You can only apply for yourself on this form or your child up to their 11th birthday. Once your child reaches the age of 11 your access to their records will cease and they will now have to consent. Further forms will need to be completed by yourself and your child. If access is given this will cease again on the day before they reach 16 and they will then need to apply for their own online access and record viewing. If applying for a child you will need to bring your photo ID and your child’s birth certificate. When the form is completed please hand it in at reception with your ID and you will be issued with your online access ID and instructions.
IMPORTANT: Our online services have changed
As part of a CCG initiative for all practices in the area to be on the same clinical (computer) system, South Street Surgery changed from Emis Web to SystmOne on 19th April 2016.
Logins can be collected during normal surgery hours at Bishop’s Park. If you wish to collect from South Street please complete the ‘ask the practice a question’ form and we will arrange for these to be delivered to South Street.
